Please note: A $20 Provider Review Fee is required at the time of completing your online intake for all Metabolic Support Programs. This fee covers the medical provider's review of your health history, intake forms and eligibility assessments of the program. Provider fee is separate from all program fees, consultation fees, laboratory fees, medication costs, and treatment costs. The $20 Provider Fee is non-refundable once your intake has been submitted for review.
